ABSTRACT

Gestational hypertension and pre-eclampsia are important causes of perinatal morbidity. The objective of the present study was to determine the increase in relative risk for developing hypertensive disorders of pregnancy based on the evaluation of pregnant women between 20 and 25 weeks of gestation, and to correlate the findings at this period with the outcome of pregnancy. We conducted a prospective cohort study, with a convenience sample of 1417 patients evaluated at this gestational age, of which 1306 were contacted at childbirth. We detected an increased relative risk of 2.69 (95%CI: 1.86 to 3.89) associated with pulsatility index of the uterine arteries, a 2.8 increase (95%CI: 1.58 to 5.03) in relative risk attributed to maternal age above 35 years, a 1.68 increase (95%CI: 1.17 to 2.40) attributed to parity greater than or equal to 3, and a 5.35 increase (95%CI: 4.18 to 6.85) attributed to chronic hypertension and obesity, with a progressive increase in relative risk according to the degree of overweight, i.e., grades 1, 2, 3, and morbid obesity (2.58, 3.06, 5.84, and 7.28, respectively).

ABSTRACT

It is still unknown whether excessive consumption of sugar-sweetened beverages may be linked to gestational hypertensive disorders, other than preeclampsia. This study investigated the association between soft drink consumption and hypertension during pregnancy, analyzing the relationship from the perspective of counterfactual causal theory. Data from pregnant women of the BRISA cohort were analyzed (1,380 in São Luis and 1,370 in Ribeirão Preto, Brazil). The explanatory variable was the frequency of soft drink consumption during pregnancy obtained in a prenatal interview. The outcome was gestational hypertension based on medical diagnosis, at the time of delivery. A theoretical model of the association between soft drink consumption and gestational hypertension was constructed using a directed acyclic graph. Marginal structural models (MSM) weighted by the inverse of the probability of soft drink consumption were also employed. Using Poisson regression analysis, high soft drink consumption (≥7 times/week) was associated with gestational hypertension in São Luís (RR=1.48; 95%CI: 1.03-2.10), in Ribeirão Preto (RR=1.51; 95%CI: 1.13-2.01), and in the two cohorts combined (RR=1.45; 95%CI: 1.16-1.82) compared to lower exposure (<7 times/week). In the MSM, the association between high soft drink consumption and gestational hypertension was observed in Ribeirão Preto (RR=1.63; 95%CI: 1.21-2.19) and in the two cohorts combined (RR=1.51; 95%CI: 1.15-1.97), but not in São Luís (RR=1.26; 95%CI: 0.79-2.00). High soft drink consumption seems to be a risk factor for gestational hypertension, suggesting that it should be discouraged during pregnancy.

ABSTRACT

A síndrome dos ovários policísticos (SOP) é uma condição endócrina frequente em mulheres em idade reprodutiva. O quadro clínico é manifesto por anovulação crônica hiperandrogênica, acompanhada muitas vezes de infertilidade; além disso, essa condição está associada ao aumento de distúrbios do metabolismo glicídico e a diversos outros riscos em longo prazo. Uma vez gestante, a mulher portadora de SOP apresenta risco aumentado em 2,8 vezes para o diabetes gestacional, em 2,0 a 4,0 vezes para o desenvolvimento de síndromes hipertensivas da gestação e em 2,3 vezes para internação em UTI neonatal. Independentemente do excesso de peso, que é comumente associado à síndrome e que certamente potencializa o risco de complicações, a SOP por si só promove alterações que cursam com a elevação dessas complicações. Esta é uma revisão narrativa sobre as potenciais complicações gestacionais relacionadas à SOP e compila a literatura mais atual sobre o tema.(AU)

Abstract Objective: To evaluate the prevalence of hypertensive disorders, perinatal outcomes (preterm infants, low birthweight infants and Apgar score < 7 at the 5th minute and fetal deaths) and the cesarean rates in pregnant women hospitalized for delivery at the Maternidade Hilda Brandão da Santa Casa de Belo Horizonte, Belo Horizonte, state of Minas Gerais, Brazil, from March 1, 2008 to February 28, 2018. Methods: A case-control study was performed, and the groups selected for comparison were those of pregnant women with and without hypertensive disorders. Out of the 36,724 women, 4,464 were diagnosed with hypertensive disorders and 32,260 did not present hypertensive disorders Results: The prevalence of hypertensive disorders was 12.16%; the perinatal outcomes and cesarean rates between the 2 groups with and without hypertensive disorders were: preterm infants (21.70% versus 9.66%, odds ratio [OR] 2.59, 95% confidence interval [CI], 2.40-2.80, p < 0.001); low birthweight infants (24.48% versus 10.56%; OR 2.75; 95% CI, 2.55-2.96; p < 0.001); Apgar score < 7 at the 5th minute (1.40% versus 1.10%; OR 1.27; 95% CI, 0.97-1.67; p = 0.84); dead fetuses diagnosed prior to delivery (1.90% versus 0.91%; OR 2.12; 95% CI, 1.67-2.70; p < 0.001); cesarean rates (60.22% versus 31.21%; OR 3.34; 95% CI, 3.14-3.55; p < 0.001). Conclusion: Hypertensive disorders are associated with higher rates of cesarean deliveries and higher risk of preterm infants, low birthweight infants and a higher risk of fetal deaths.


Resumo Objetivo: Avaliar a prevalência dos distúrbios hipertensivos, resultados perinatais (recém-nascidos pré-termo, recém-nascidos de baixo peso, índice de Apgar < 7 no 5° minuto e óbitos fetais) e as taxas de cesarianas nas gestantes internadas para assistência ao parto na Maternidade Hilda Brandão da Santa Casa de Belo Horizonte, Belo Horizonte, MG, Brasil, no período de 1° de março de 2008 a 28 de fevereiro de 2018. Métodos: Foi realizado um estudo analítico, observacional, longitudinal. Os grupos selecionados para comparação foram gestantes com e sem distúrbios hipertensivos. Do total de 36.724 gestantes, 4.464 foram diagnosticadas com distúrbios hipertensivos e 32.260 não apresentavam distúrbios hipertensivos. Resultados: A prevalência dos distúrbios hipertensivos foi de 12,16%; Os resultados perinatais e as taxas de cesarianas entre os 2 grupos de gestantes com e sem distúrbios hipertensivos foram: recém-nascidos pré-termo (21,70% versus 9,66%; odds ratio [OR] 2,59; intervalo de confiança [IC] 95%, 2,40-2,80; p < 0,001); recém-nascidos de baixo peso (24,48% versus 10,56%; OR 2,75; IC 95%, 2,55-2,96; p < 0,001); índice de Apgar < 7 no 5° minuto (1,40% versus 1,10%; OR 1,27; IC 95%, 0,97-1,67; p = 0,084); fetos mortos diagnosticados previamente ao parto (1,90% versus 0,91%; OR 2,12; IC 95%, 1,67-2,70; p < 0,001); taxas de cesarianas (60,22% versus 31,21%; OR 3,34; IC 95%, 3,14-3,55; p < 0,001). Conclusão: Os distúrbios hipertensivos estão associados a maiores taxas de cesarianas, ao maior risco de recém-nascidos pré-termo, recém-nascidos de baixo peso e a um maior risco de óbitos fetais.

Abstract Purpose To evaluate the impact of the presence of criteria for severe maternal morbidity and maternal near miss associated with hypertensive disorders on maternal and perinatal outcomes in a maternity school. Methods The present is a sub-analysis of a larger study involving 27 centers in Brazil that estimated the prevalence of serious maternal morbidity and near miss. It is an analytical and cross-sectional study with a quantitative approach, involving 928 women who were cared for at Maternidade Escola Assis Chateaubriand (MEAC, in Portuguese), Universidade Federal do Ceará (UFC, in Portuguese), from July 2009 to June 2010. The women were diagnosed with near miss according to the World Health Organization (WHO) criteria. The sample was divided into 2 groups: patients with (n = 827) and without hypertension (n = 101). The results were considered statistically significant when p < 0.05. The Pearson chi-squared and Fisher Exact tests were used for the categorical variables, and the Mann-Whitney U test was used for the continuous variables. Results In total, 51 participants with maternal near miss criteria were identified, and 36 of them had hypertensive disorders. Of these, 5 died and were obviously excluded from the near miss final group. In contrast, we observed 867 cases with non-near miss maternal morbidity criteria. During this period, there were 4,617 live births (LBs) in the institution that was studied. Conclusion In the severe morbidity/maternal near miss population, the presence of hypertensive complications was prevalent, constituting a risk factor for both the mother and the fetus.


Resumo Objetivo Avaliar o impacto da presença de critérios de morbidade materna grave e potencial evento adverso materno associados a distúrbios hipertensivos nos desfechos maternos e perinatais em uma maternidade escola. Métodos Trata-se de uma subanálise de um estudo maior, envolvendo 27 centros, que estimou a prevalência de morbidade materna grave e potencial evento adverso no Brasil. Realizou-se um estudo analítico e transversal, com abordagem quantitativa, envolvendo 928 mulheres atendidas na Maternidade Escola Assis Chateaubriand (MEAC), na Universidade Federal do Ceará (UFC), no período de julho de 2009 a junho de 2010, diagnosticadas com potencial evento adverso de acordo com os critérios da Organização Mundial da Saúde (OMS). A amostra foi dividida em dois grupos: pacientes com (n = 827) e sem hipertensão (n = 101). Os resultados foram considerados estatisticamente significativos quando p < 0,05. O teste do Qui-quadrado de Pearson e o teste exato de Fisher foram usados para as variáveis categóricas, e o teste U de Mann-Whitney, para as variáveis contínuas. Resultados Foram identificados 51 participantes com critérios de potencial evento adverso materno, sendo 36 mulheres com distúrbios hipertensivos. Destas, 5 morreram e foram obviamente excluídas do grupo final do potencial evento adverso. Foram observados 867 casos com critérios de morbidade materna que não caracterizavam potencial evento adverso. Nesse período, houve 4.617 nascidos vivos (NVs) na instituição estudada. Conclusão Na população com morbidade grave/potencial evento adversomaterno, a presença de complicações hipertensivas é prevalente, constituindo fator de risco para o binômio materno-fetal.

ABSTRACT

RESUMEN Los desórdenes hipertensivos del embarazo se encuentran dentro de las tres primeras causas de morbimortalidad materna y perinatal a nivel mundial, hasta el año 2016. Diferentes estudios realizados en los últimos 5 años e importantes organizaciones científicas han abordado este tema en el que existen discrepancias en cuanto a su etiopatogenia. Sin embargo, los avances logrados en la misma, la profundización en sus manifestaciones clínicas, los modos de presentación y los elementos diagnósticos han permitido el reconocimiento precoz y la efectividad del tratamiento. Esto ha ocasionado, principalmente en lo referente a los desórdenes hipertensivos tipo I, cambios que afectan desde la definición hasta el tratamiento. De esta manera, el presente documento pone al alcance de la comunidad médica una amplia revisión científica que facilita un mejor diagnóstico de la preeclampsia y de otras formas de hipertensión arterial en la etapa grávido-puerperal. Asimismo, contribuirá a reducir el error diagnóstico y logrará una intervención temprana para la obtención de mejores resultados maternos y perinatales (AU).


SUMMARY Hypertensive disorders of pregnancy are among the first three causes of maternal and perinatal Morbimortality in the world until 2016. Important scientific organizations and several studies carried out in the last five years have approached the theme, but there are still discrepancies with respect to etiopathogeny. Nevertheless, advances in it and deepening in its clinical manifestations, presentation ways and diagnostic elements have allowed its precocious recognition and diagnostic elements, causing changes from its definition up to its treatment, mainly in the case of the hypertensive disorders type I. In the current article, the authors put at reach of the medical community a wide scientific review facilitating a better diagnosis of preeclampsia and other forms of arterial hypertension in the pregnancy-puerperal stage that will contribute to reducing diagnosis error and making an early intervention, leading to better maternal and perinatal results (AU).

Resumen Antecedentes: La preeclampsia se presenta en el 8% de los embarazos y genera el 25% de las muertes perinatales. Aunque su etiología es multifactorial, algunas alteraciones metabólicas se asocian a la disfunción endotelial presente en la enfermedad, y su estudio podría identificar marcadores tempranos de daño. Objetivo: Determinar la relación existente entre concentración plasmática de triglicéridos en las pacientes embarazadas con enfermedad hipertensiva inducida por el embarazo. Método: Estudio prospectivo que incluyó 147 pacientes gestantes sanas y 120 mujeres con enfermedad hipertensiva inducida por el embarazo y se determinaron colesterol total, lipoproteínas de baja densidad (LDL), lipoproteínas de alta densidad (HDL), hemoglobina glucosilada (HbA1c), biometría hemática (BH), química sanguínea (QS), electrolitos séricos, nitritos séricos y pruebas de función hepática. Resultados: Se encontró una diferencia significativa en la concentración de triglicéridos entre las pacientes normotensas y con preeclampsia (261.22 ± 80.27 vs. 361.46 ± 135.17 mg/dl; p < 0.0008). Además, se observó una menor concentración sérica de nitritos en las pacientes con preeclampsia, que contribuye a explicar la vasoconstricción. Conclusiones: Se encontró una asociación entre la hipertrigliceridemia y la presencia de DM II con el desarrollo de la enfermedad hipertensiva inducida por el embarazo.


Abstract Background: Preeclampsia occurs in 8% of pregnancies and generates 25% of perinatal deaths. Although its etiology is multifactorial, some metabolic alterations are associated with the endothelial dysfunction present in the disease, and its study could identify early markers of damage. Objective: To determine the relationship between plasma concentration of triglycerides in pregnant patients with hypertensive disease induced by pregnancy. Methods: Prospective study that included 147 healthy pregnant women and 120 women with hypertensive disease induced by pregnancy. Total cholesterol, low-density lipoprotein, high-density lipoprotein, Hemoglobin A1c, triglycerides, BH, QS, serum electrolytes, serum nitrites and liver function tests were determined. Results: A significant difference was found in the concentration of triglycerides between normotensive and patients with preeclampsia (261.22 ± 80.27 vs. 361.46 ± 135.17 mg/dl, p < 0.0008). In addition, a lower serum concentration of nitrites was observed in patients with preeclampsia, which helps explain vasoconstriction. Conclusions: We found an association between hypertriglyceridemia and the presence of diabetes mellitus II with the development of hypertensive disease induced by pregnancy.

La enfermedad hipertensiva gravídica constituye una patología obstétrica relativamente frecuente. En los últimos años se ha avanzado en la comprensión de la patogenia de la enfermedad, al ser el sistema inmune uno de los actores de vital importancia en la aparición de este trastorno. Las células asesinas naturales de la decidua constituyen un componente fundamental en el remodelado vascular durante la formación de la placenta. Cuando existen alteraciones en esta población celular, se induce una isquemia placentaria que promueve la liberación de factores vasculares y mediadores de la inflamación que promueven el daño sistémico con aparición de los síntomas de la enfermedad(AU)


Pregnant hypertension is a frequent pregnancy disorder. Researchers has recentely advanced in understanding this disease pathogenesis. The immune system plays a key role in this disease onset. Decidual natural killer cells are fundamental component in vascular remodeling during placental development. When alterations occurs in this cell population, placental ischemia is induced promoting the release of vascular factors and inflammation mechanisms that cause these disease symptoms(AU)

La hipertensión arterial de causa adrenal asociada al embarazo es infrecuente. Su presencia genera una elevada morbilidad y mortalidad materna y fetal. Los cambios de la fisiología endocrina que acontecen en esta etapa, modifican el cuadro clínico y bioquímico de las enfermedades adrenales causantes de hipertensión. El objetivo de esta revisión es brindar información actualizada sobre el hipercortisolismo endógeno, el aldosteronismo primario y el feocromocitoma en la gestación -a los profesionales de la salud vinculados con la atención a embarazadas- que les permita diagnosticar y tratarlas temprana y adecuadamente mediante la aplicación del método clínico. Se revisa la literatura y se exponen los avances médicos en el tema(AU)


Blood hypertension of adrenal origin associated to pregnancy is unfrequent and generates high maternal and fetal morbidity and mortality. Changes in the endocrine physiology during this phase modify the clinical and biochemical picture of hypertension-causing adrenal diseases. The objective of this review was to provide health professionals in charge of pregnancy care with updated information on endogenous hypercortisolism, primary aldosteronism and pheochromocytoma in pregnancy. This will allow them to diagnose and treat this disorder early and adequately by means of the clinical method. Relevant literature was reviewed and the latest medical advances in this field were presented(AU)

OBJECTIVE:The aim of the study was to identify the frequency of risk factors for hypertensive disorders in pregnancy in Southern Brazil. METHODS: The study included 161 patients with hypertensive disorders and 169 control subjects matched by age and ethnicity. The frequency of the risk factors was compared by Fisher's exact test, chi-square and Student's t test. A multivariate logistic regression analysis assessed the independent role of clinical, social and demographic factors which were associated with occurrence of the hypertensive disease in pregnancy in the univariate analysis. RESULTS: Patients enrolled in the study were predominantly Caucasian (73 percent) and the mean age was 29. In the multivariate analysis, the variables associated were: family history of preeclampsia (p = 0.001; OR = 3.88; 95 percent CI = 1.77-8.46), diabetes (p = 0.021; OR = 3.87; 95 percent CI = 1.22-12.27) and chronic hypertension (p = 0.002; OR = 7.05; 95 percent CI = 1.99-24.93). CONCLUSION: The risk factors associated with hypertensive disorders in pregnancy appear to be similar to those reported in other countries. The knowledge of the risk factors could be helpful in a prenatal care.


OBJETIVO: Identificar a frequência dos fatores de risco para distúrbios hipertensivos durante a gravidez na região Sul do Brasil. MÉTODOS: O estudo incluiu 161 pacientes com distúrbios hipertensivos e 169 controles, compatíveis em idade e etnia. A frequência dos fatores de risco foi comparada a partir do teste exato de Fisher, teste qui-quadrado e teste t de Student. Uma análise logística multivariacional de regressão avaliou a influência de fatores clínicos, sociais e demográficos, associados com a ocorrência de doenças hipertensivas durante a gravidez na análise univariada. RESULTADOS: Os pacientes envolvidos no estudo eram predominantemente caucasianos (73 por cento) e a idade média foi 29 anos. Na análise multivariada as variáveis associadas foram: histórico de pré-eclâmpsia na família (p = 0,001; OR = 3,88; 95 por cento IC = 1,77-8,46), diabetes (p = 0,021; OR = 3,87; 95 por cento IC = 1,22-12,27) e hipertensão crônica (p = 0,002; OR = 7,05; 95 por cento IC = 1,99-24,93). CONCLUSÃO: Os fatores de risco associados a distúrbios hipertensivos durante a gravidez parecem ser similares àqueles relatados em outros países. O conhecimento sobre os fatores de risco pode ser útil durante o acompanhamento pré-natal.

ABSTRACT

A população de obesos entre as mulheres na menacme tem aumentado de maneira assustadora nos últimos anos. A consequência é o aumento da incidência de gestantes obesas com elevado risco cardiometabólico e sujeitas a intercorrências perigestacionais. A assistência pré-natal nesse grupo de gestantes é precária devido à falta de equipe multidisciplinar preparada e material adequado. O profissional que assiste a gestante obesa deve ter como objetivo principal prevenir e intervir em tempo hábil nas intercorrências às quais mãe e feto estão sujeitos durante o período perigestacional. Ele deve estar livre de preconceitos e tentativas exageradas e inadequadas de perda de peso durante a gestação da paciente. O alvo principal da abordagem multidisciplinar é o preparo da gestante para mudança de seu estilo de vida familiar sabendo que a obesidade é uma doença de transmissão interpessoal de hábitos. A associação de riscos potenciais para mãe e feto à obesidade é bem estabelecida, mas o conhecimento desses riscos e a condução da gestação de maneira adequada são pouco praticados. O presente artigo buscou contribuir para melhora da assistência pré-natal das gestantes obesas juntamente com a criação de grupos multidisciplinares para condução dessas pacientes.


The obese population between women in the mecname has been grown in a concerned manner in the past few years and its consequence is the growth of the obese pregnant patients with high cardio metabolic risk and subject to clinical intercurrences. The prenatal assistance in this pregnant group is precarious due to a lack of a prepared multidisciplinary group and also an adequate work material. The professional whom takes care of an obese patient shall be as the main purpose the prevention and intervention in a timely manner in the clinical intercurrences which mother and fetus are subject to during the gestational period. His method shall be free of prejudices, inadequate and exaggerated attempts to loss of weight during the patient pregnancy. The main target of the multidisciplinary approach is the pregnant patient's prepare to a new familiar lifestyle keeping in mind that obesity is a sickness of interpersonal habit transmission. The association of potential risks to mother and fetus during obesity is well determined but the knowledge of those risks and the pregnancy conduction in a proper manner are few practiced. The present article wanted to contribute to an improvement of prenatal assistance of obese pregnant jointly with a multidisciplinary group creation to conduct those patients.

FUNDAMENTO: Os fatores de risco para as síndromes hipertensivas gestacionais (SHG) são vários e podem relacionar-se à região e etnia da população. No Estado de Goiás, são escassos os estudos sobre esse tema. OBJETIVO: Investigar os fatores de risco maternos para SHG. MÉTODOS: Utilizou-se um estudo caso-controle por meio da análise dos prontuários das parturientes da Maternidade do Hospital das Clínicas da Universidade Federal de Goiás (HC-UFG), em 2005. Os fatores de risco foram analisados pela análise de regressão logística e pelo teste exato de Fisher. RESULTADOS: Em 2005, houve 890 partos na Maternidade do HC-UFG, e 129 gestantes apresentaram diagnóstico de SHG (14,5 por cento). A análise multivariada identificou a obesidade como fator de risco tanto para hipertensão gestacional (HG) (OR: 17,636; IC 95 por cento: de 2,859 a 108,774) como para hipertensão arterial crônica superajuntada à pré-eclâmpsia (HCSPE) (OR: 27,307; IC 95 por cento: de 4,453 a 167,440). Primiparidade constituiu fator de risco para HG (OR: 5,435; IC 95 por cento: de 1,923 a 15,385). Idade acima de 30 anos foi fator de risco para HCSPE (OR: 5,218; IC 95 por cento: de 1,873 a 14,536) e fator de proteção para pré-eclâmpsia (PE) (OR: 0,035; IC 95 por cento: de 0,003 a 0,364). Raça não-branca representou risco independente para PE (OR: 13,158; IC 95 por cento: de 1,672 a 100,000) e PE prévia para HCSPE (OR: 4,757; IC 95 por cento: de 1,261 a 17,938). Das gestantes com hipertensão arterial crônica (HAC), 73,5 por cento desenvolveram HCSPE (p < 0,001). CONCLUSÃO: Os fatores obesidade, raça não-branca, PE prévia, idade acima de 30 anos e HAC identificados foram semelhantes à maioria dos achados da literatura.


BACKGROUND: There are several risk factors for hypertensive syndromes in pregnancy (HSP), and these can be related to regional and ethnic factors. Studies on this issue are scarce in the State of Goiás. OBJECTIVE: To investigate maternal risk factors for HSP. METHODS: Case-control study based on the analysis of medical records of parturients from the Maternity Service of Hospital das Clínicas da Universidade Federal de Goiás (HC-UFG) in 2005. Risk factors were analyzed using Logistic Regression Analysis and Fisher's Exact Test. RESULTS: In 2005, there were 890 deliveries in the Maternity Service of HC-UFG, and 129 pregnant women were diagnosed with HSP (14.5 percent). The multivariate analysis identified obesity as a risk factor both for gestational hypertension - GH (OR: 17.636; 95 percent CI: 2.859 to 108.774) and for preeclampsia superimposed on chronic hypertension - PESCH (OR: 27.307; 95 percent CI: 4.453 to 167.440). Primiparity was a risk factor for GH (OR: 5.435; 95 percent CI: 1.923 to 15.385). Age above 30 years was a risk factor for PESCH (OR: 5.218; 95 percent CI: 1.873 to 14.536) and a protective factor against preeclampsia - PE (OR: 0.035; 95 percent CI: 0.003 to 0.364). Non-white race was an independent risk for PE (OR: 13.158; 95 percent CI: 1.672 to 100.000) and previous PE for PESCH (OR: 4.757; 95 percent CI: 1.261 to 17.938). Of the pregnant women with chronic hypertension (CH), 73.5 percent developed PESCH (p < 0.001). CONCLUSION: The factors identified - obesity, non-white race, previous PE, age above 30 years and CH, were similar to those found in the majority of studies in the literature.

ABSTRACT

OBJETIVO: Quantificar a prevalência de HG em adolescentes primíparas; definir fatores de preditividade para a ocorrência de HG e sua repercussão nos recém-nascidos. MÉTODOS: Foram acompanhadas 29 adolescentes primíparas durante o pré-natal até a 12ª semana de puerpério, com idade média de dezesseis anos, do ambulatório do Serviço de Adolescentes da Maternidade Escola Assis Chateaubriand (MEAC) da Universidade Federal do Ceará (Fortaleza). As gestantes foram dividas em dois grupos; ou seja, nas que permaneceram normotensas (Grupo I) e naquelas que desenvolveram hipertensão gestacional -HG - (Grupo II). As variáveis investigadas na avaliação de valor de preditividade no desenvolvimento de HG foram valores antropométricos, aspectos socioeconômicos, o hábito de fumar, a hereditariedade para HAS (pai/mãe), os exames do pré-natal solicitados na primeira consulta do pré-natal e a microalbuminúria e a monitorização ambulatorial da pressão arterial (MAPA) na 28ª semana de gestação. As gestantes foram acompanhadas no parto e no puerpério tardio (12ª semana de puerpério). Os recém-nascidos das mães do nosso estudo foram avaliados ao nascer pelos índices de APGAR e de Capurro, peso, estatura e pela presença de hipóxia perinatal. RESULTADOS: A prevalência de HG foi de 51,7 por cento . A hereditariedade para HAS apresentou-se com o maior valor de preditividade para HG com odds ratio de 10,99. A pressão arterial diastólica maior ou igual a 70 mmHg, na idade média gestacional de 35 semanas, apresentou significância estatística como valor de preditividade para HG. Na MAPA encontramos valor de preditividade para HG: carga pressórica diastólica em vigília, carga pressórica sistólica e diastólica no sono noturno, variabilidade pressórica e pressão máxima diastólica no sono. OBJECTIVE: To quantify PH prevalence in primiparous adolescents; define predictive factors for the occurrence of PH and its impact on newborns. METHODS: We followed 29 primiparous adolescents from the prenatal period through the 12th week of the puerperium, with a mean of sixteen years of age, served at the Outpatient Facility for Adolescents of Maternidade Escola Assis Chateaubriand (MEAC) of Universidade Federal do Ceará (Fortaleza, Brazil). The pregnant adolescents were divided into two groups, that is, those who remained normotensive (Group I) and those who developed PH (Group II). The variables investigated in the assessment of the value of predictability for the development of PH were anthropometric measures, socioeconomic aspects, smoking habit, inheritance for SAH (father/mother), prenatal tests requested in the first prenatal care visit in addition to microalbuminuria and ambulatory blood pressure monitoring (ABPM) in the 28th week of gestation. The pregnant adolescents were followed up at delivery and late puerperium (12th week after the puerperium). The newborns to the mothers included in our study were assessed at birth according to the Apgar score and the Capurro method, for weight, height and perinatal hypoxia. RESULTS: The prevalence of PH was 51.7 percent. Inheritance for SAH presented the highest predictive value for PH with an odds ratio of 10.99. Diastolic arterial pressure equal to or above 70 mmHg at the gestational age of 35 weeks was statistically significant as a predictive value for PH. At ABPM we found a predictive value for PH: diastolic pressure load during alertness, diastolic and systolic pressure load during night sleep, pressure variability and maximum diastolic pressure during sleep. ABSTRACT

This study was conducted in Mymensingh Medical College and Hospital, Mymensingh. It lasted for one year commencing in July 2003, and was designed to find out and compare the outcomes of gestational diabetes mellitus (GDM) with a view to reducing the complications. Forty cases of GDM with single pregnancy were selected as experimental group, while forty other cases of pregnant women without GDM constituted the control group. Questionnaire and observation chart were the research instrument in this study. Statistical analyses were done manually. It was found that the GDM mothers had a higher risk of complications like pre-eclampsia, hypertension, urinary tract infection and cesarean delivery. On the basis of these findings, however, appropriate measures for prevention, treatment and management of GDM may be adopted for better pregnancy outcomes.

ABSTRACT

OBJETIVO: Avaliar o valor prognóstico da proteinúria nas gestantes com síndromes hipertensivas nos desfechos maternos e perinatais. MÉTODOS: Estudo transversal retrospectivo de 334 gestantes com síndromes hipertensivas que pariram no Hospital São Paulo na disciplina de Obstetrícia da UNIFESP/EPM, no período de 1° de janeiro de 1999 a 31 de dezembro de 2002. RESULTADOS: Após a revisão dos prontuários, as pacientes foram divididas em quatro grupos: I sem proteinúria (n-203), II com proteinúria de 0,3 a 1g (n-39), III de 1 a 2g (n-45) e grupo IV de 2g ou mais (n-47). Na ausência da proteinúria houve um caso de descolamento prematuro da placenta. Com proteinúria observou-se desfechos maternos adversos, com a presença das complicações, proporcional à elevação da proteinúria, sendo a síndrome HELLP a mais freqüente com 30,5 por cento (40/131) seguida da eclâmpsia com 3,8 por cento (5/131), DPP 3,01 por cento (4/131) e insuficiência renal 0,7 por cento (1/131). Foi constatado um óbito materno nesse grupo, perfazendo-se o CMM de 763100.000/n.v. Em relação aos desfechos perinatais, no grupo sem proteinúria não houve elevação dos efeitos adversos. Na presença da proteinúria e a elevação dos seus níveis observou-se pior prognóstico perinatal com os seguintes indicadores: aumento da prematuridade (62,2 por cento vs 11,5 por cento), recém-nascidos com peso < 2500g (6,5 por cento vs 1,5 por cento), Apgar < 7 no 5° minuto (30,4 por cento vs 3,5 por cento), restrição de crescimento intra-útero (41,9 por cento vs 6,5 por cento), cuidados intensivos na unidade neonatal (59,8 por cento vs 15,5 por cento), natimortos (14,4 por cento vs 1,4 por cento), e óbitos neonatais (6,1 por cento vs 0,98 por cento). O coeficiente de mortalidade perinatal foi maior com proteinúria (175 vs 19,7) e, quando > 2g (297,8 vs 19,7). CONCLUSÕES: A presença da proteinúria e a elevação dos seus níveis, aumentaram as complicações maternas, principalmente a síndrome HELLP e eclâmpsia. Observou-se incidência de complicações perinatais com elevação significativamente maior da prematuridade, recém-nascidos com Apgar < 7, peso < 2500g, CIUR, natimortos e óbitos neonatais.
